You will receive this from the Mission Team and fill it out electronically.

 

You’re in charge!

The Wish Consent & Liability Publicity Release Form lets us know if you’re okay with us sharing your wish story. The three options on the form give permission for different levels of media. Don’t hesitate to reach out if you have questions about the options. Sometimes, news outlets want to do stories about our amazing wish kids and their families. Please know it’s always up to you if you want to participate in media opportunities like these when they come up.
